Is there an alumni you believe deserves to be inducted into the Humboldt Athletic Hall of Fame?

Complete the following form and the committee will review your nomination.

Criteria for nomination:

Must be a Humboldt graduate
Must have graduated at least 15 years ago
Nominee must agree to accept the nomination and be present at the induction ceremony unless an unforeseeable event occurs, preventing their attendance.
Nominee must have been an All-Conference, Or All-Conference Honorable Mention, Or All-State, Or All-State Runner Up, or All-State 1st/2nd Team athlete OR have a track record of helping support Humboldt athletics over the years and made an impact within the Humboldt Community.

The banquet will be held Annually in September, at Humboldt High. If the person you are nominating is selected they will need to be there in person to accept the award.
